Why your Headway claims get denied

The four denial patterns that show up most often when a behavioral health practice bills through a platform, and what each one costs.

Headway can make claims submission easier, but it cannot remove every payer rule. Three problems deserve a close look: benefit carve-outs, timely-filing limits, and credentialing mismatches. Each can produce a denial even when the session was covered and documented correctly.

A behavioral health carve-out occurs when a patient's mental health benefits are administered by a different company from the medical plan shown on the insurance card. If eligibility is checked only against the medical payer, the claim may go to the wrong destination. Confirm the behavioral health administrator, electronic payer ID, and authorization rules before the first session. Recheck when coverage changes.

Timely filing is the payer's deadline for receiving a clean claim. The window may be 90 days, 180 days, or another period set by contract. A platform queue can make a claim look active while corrections consume that window. Track the date of service, first submission date, rejection date, and corrected submission date. A clearinghouse rejection is generally not proof that the payer received the claim.

Credentialing mismatches create another gap. A clinician may be approved with the platform but not effective with a specific payer, product, tax identification number, or practice location on the date of service. Compare the claim's billing and rendering details with the payer's roster confirmation. Do not rely on a general network status label.

These issues matter beyond one platform. Experian Health's State of Claims 2025 found that 41% of providers report that more than 10% of their claims are denied. The useful control is a claim-level timeline that connects eligibility, credentialing, submission, adjudication, and deposit. Platform dashboards show part of that chain. Your ledger should show all of it.
