CO-16
Claim lacks information or has submission/billing errors
Contractual obligation; provider write-off unless appealed
- What it means
- Something required is missing or malformed. CO-16 almost always travels with a RARC that names the actual missing element.
- Likely cause
- Missing NPI or taxonomy, bad subscriber ID, missing rendering provider, or an incomplete supervising-provider loop for associate clinicians.
- How to fix it
- Read the accompanying RARC on the 835, not just the CARC. Correct the named field, and resubmit. Track which RARC pairs with CO-16 most often.
- Revenue impact
- Fully recoverable, but it is the single most common reason claims sit unworked.
- missing information
- enrollment

Seeing a lot of these?
Repeated CO-16 adjustments usually point to a step before submission. First Pass Paid is building pre-submission checks and a denial worklist to catch these earlier.
