CO-29
The time limit for filing has expired
Contractual obligation; provider write-off unless appealed
- What it means
- The claim arrived after the payer's timely-filing deadline.
- Likely cause
- Claims that sat in a work queue, a credentialing gap that delayed submission, or denials that were never reworked before the clock ran out.
- How to fix it
- Appeal with proof of timely filing (clearinghouse acceptance reports). Then fix the upstream aging report so it cannot happen again.
- Revenue impact
- Mostly unrecoverable. This is pure lost revenue and the clearest case for checks before submission.
- timely filing
- write-off
- aging

Seeing a lot of these?
Repeated CO-29 adjustments usually point to a step before submission. First Pass Paid is building pre-submission checks and a denial worklist to catch these earlier.
